Congratulations on your new job! Please follow the simple steps below to obtain a work permit. Once your permit application has been fully completed, you may drop it off to Mrs. Miklavic in room 139. Should the room be closed, feel free to drop it off in the Main Office for Mrs. Miklavic’s attention. It is recommended to provide at least 24 hours for Employment Permits to be processed.
Follow the directions on the Permit Application according to your age. Requirements differ depending on the student’s age.
Bring your application to your employer. Your employer must complete and sign the Promise of Employment section. If possible, please have your employer attach a business card with all contact information on it.
Complete the relevant sections on the remainder of the Permit Application.
Submit the completed form to Mrs. Miklavic in room 139.
Obtain Employment Permit from Mrs. Miklavic in room 139.
Provide the original copy of the Employment Permit to your employer.
**OUT-OF-DISTRICT STUDENTS** If you live in Mendon or Upton and need to obtain a work permit, you will need to take the following steps:
Follow the directions on the Permit Application according to your age. Requirements differ depending on the student’s age.

Complete the relevant sections on the remainder of the Permit Application.
Submit the completed form to Mrs. Miklavic (please call the school or email [email protected] to make an appointment). In addition to the permit application, you will also need to provide:
Proof of residency (license or parent license is fine)
A current school transcript
A copy of the student's birth certificate
Mrs. Miklavic will be able to complete the permit at the time of your appointment, however, the student must be present to sign the work permit upon completion.
Provide the original copy of the Employment Permit to your employer.
